Virtual reality to find memory problems early

Some older people have small problems with memory and thinking. Doctors call this mild cognitive impairment, or MCI.

About 15% to 20% of people over 65 have MCI. About half of them can develop dementia, like Alzheimer's, in up to five years.

Roberto Aquino is a scientist at USP, a university in São Carlos. In his project, people do tasks in virtual reality, a digital world. Machines record what the brain and the eyes do.

Then artificial intelligence (AI) studies the data and explains its answer. It helps the doctor, but it does not replace the doctor.

The project is not finished. After more tests, the team wants to share the software with scientists everywhere.

USP researcher combines virtual reality and AI to detect cognitive decline early

A researcher at the University of São Paulo (USP) in São Carlos is developing a platform to detect mild cognitive impairment (MCI) early. MCI is a stage between normal aging and early dementia. An estimated 15% to 20% of people over 65 have it, and about half of them may develop dementia, such as Alzheimer's, within five years.

Roberto Douglas Guimarães de Aquino, a postdoctoral researcher supervised by Professor Alexandre Delbem, leads the project. Participants complete tasks in immersive virtual reality while an EEG records their brain activity and an eye tracker records their eye movements.

AI models then analyze the data. Aquino said the algorithm does more than give an automated answer: it explains why. Its report supports the neurologist's decision but does not replace the doctor.

The work, funded by FAPESP, has not been clinically validated yet. After validation, the software, algorithms and data will be released as open source. Delbem said scientists around the world could then collaborate and replicate the method.

Virtual reality and an AI that explains itself: spotting cognitive decline early

Most medical AI tools are judged by whether they get the answer right. A project at the University of São Paulo (USP) in São Carlos puts just as much weight on a second question: can the machine explain why?

The target is mild cognitive impairment (MCI), a stage between normal aging and early dementia. An estimated 15% to 20% of people over 65 have it, and about half may progress to dementia, such as Alzheimer's, within five years, which makes early detection all the more valuable.

The platform is the postdoctoral research of Roberto Douglas Guimarães de Aquino, under Professor Alexandre Delbem, coordinator of the FAPESP-backed ciaGsaúde center. Aquino acknowledged that conventional tests have important limitations and described his approach as a paradigm shift.

In a single session, participants work through cognitive tasks in immersive virtual reality while an EEG tracks brain activity and an eye tracker follows their gaze. Machine learning models then sift through the data, with a deliberate emphasis on interpretability. The output is a report that supports the neurologist rather than replacing them.

Clinical validation is still pending. Once it is done, the software, algorithms and datasets are due to be released as open source, so that, in Delbem's words, scientists worldwide can replicate the Brazilian methodology.

An answer you cannot explain is hard to trust, in medicine and in language learning alike. Knowing why a sentence sounds right is what turns a lucky guess into real understanding.
